When a patient comes in with an apt, check office policy regarding such situations. In order for an appointment determination to be made, it is advisable to obtain the most precise assessment of the patients medical problem and ask the physician whether to work the patient into the days schedule. Unscheduled visits at peak hours by sales represenatives or phamaceutical reps can result in wasted time for staff and reps. May want to block off one or two quiet periods in the middle of the week, perhaps around the noon hour, for this type of business call. sales person will have enough time to show products and no patient care time will be lost.
emergency situations
Assess the problem and then take on or more of the following actions:
-refer patient to a hospital emergency facility.
-suggest
the patient come to the office immediately.
-schedule the patient for on of the appointment periods left open for urgent or emergent problems.
-work the patient into the scheduleand enter the appointment with a colored pen or pencil.
patient flow
scheduling methods depends on the size and specialty of the practice, the number of physicians in the practice, and the doctors preference, consider these factors and the goal of maintaining smooth patient flow with a minimum of waiting time as you evaluate the following scheduling systems.
steam schedule
gives each patient a specific appointment time. Patients seen for the first time are usually assigned 45-60 min apts, whereas follow up examinations are scheduled every 10,15 or 20 min. The time allotment will vary depending on the physicians specialty and the circumstances.
clustered appointments
sets aside blocks of time for similar patient problems, especially when it is known that several patients will require the same services.
single book appointments
used when an appointment may take a great amount of time.
double book appointments
technique that allows two or more patients to have an appointment for a particular time, depending on the proper combination of patient complaints. this system is used to work in emergencies and book in an already full schedule but involves risking the goodwill of patients if they must always wait to see the dr.
true wave scheduling
a flexible appointment system that attempts to keep patient flow moving smoothly from one hour to the next. all patients are told their appointments are on the hour and each is seen in the order they arrive.
modified wave scheduling
patients are schduled at the first half of each hour with single 10 minute appointments, no appointments are scheduled the second half of the hour allowing for walk ins.
open access
is called same day scheduling, same day access, or advanced access, allows most patients to obtain appointments the same day they call.
open hours
allows patients to walk in anytime within a specified time frame, they sign in with the receptionist and are seen in order of their arrival.

preoperative /postoperative patient appointments
preoperative appointment the week of surgery and makes appointments for laboratory tests and and x rays.
Postoperative appointment is typically scheduled at the time surgery arrangements are made.
no show patient/cancelled patient
patient who misses a first
appointment might be called at the end of the day to suggest rescheduling the visit; after a second missed appointment, an immediate call might be made to find out the reason and to remind the patient that a third no show would result in dismissal by the physician. after a third appointment is missed, a certified letter should be sent to release the patient.
If a patient cancels and appointment, and does not set up an alternative time, the name should be listed on a cancellation sheet for
tracking purposes or remain on the appointment page with a line drawn through it and a notation in the margin to indicate a cancellation was requested by the patient.
Some offices charge for failure to keep an appointment.
appointment reference sheet
at the end of the previous day or before the first patients arrive each day, the medical assistant prepares a schedule of patients to be seen by the physician. Can be either printout, a photocopy of the daily appointment sheet, or a typed list of names, times and procedures. Additional copies should be made for additional staff members. The reference sheet enables the physician to preview the day and recall established patients in order to prepare for each patients visit.
